| STEP 1, Fax or Email Listed Items - |
Click on the links below to view the HTML pages(or word documents), then print out the pages, fill in all the fields with a pen, and scan in jpg, gif, or bmp format so you can e-mail or fax them to us....

3. TWO Forms of I.D.
4. Copy of Social Security Number
5. W9 Form
FOLLOW CAREFULLY...
* Document can NOT be altered in any way.
* Please print clearly when filling out contract. Do NOT type!!!
* The Models Agreement must be filled out exactly as is shown in the Agreement Example.
* If you live outside of the US or Canada please include a copy of the agreement in your native language Click Here for list of contracts.
* Examples of good forms of I.D. are drivers license, passport, birth certificate,
* Both forms of I.D. must be scanned and emailed ONLY. Do Not Fax I.D. or it will be rejected.
* US models must send a scan of Social Security Number or a piece(example - drivers license) that displays the number. Written social numbers will be rejected. |

After you have emailed or faxed us all of the above documents, and recieved a confirmation from us that they are correct, you must then mail the following original documents to us at....

FOLLOW CAREFULLY...
* You must MAIL the original signed documents, no copies!
* If documents have not been received within 3-4 weeks of your activation date. Your account will be suspended till arrival of your documentation.
*You will first email or fax us copies of the above documents before mailing us the originals. This way we can verify the information is filled in correctly. |
